Opportuity Details:

• Schedule: Monday-Friday, 8a-5p
• Daily Volume: 10 admissions
• Patient Population: Geriatric
• Duties: Managing chronic conditions, preventative care & assessments, medication management, and care coordination
• Support Staff: MA and RN
• EMR: Cerner
• Must be board-certified
• Paid travel and expenses
